DISABLED PERSON’S EXEMPTION DOCUMENTATION REQUIRED

- One of the following documents
- Class 2 Disabled Person Illinois Identification Card from the Illinois Secretary of State’s Office.
- Proof of Social Security Administration disability benefits. This proof includes an award letter, verification letter, or annual cost of living adjustment.
- Proof of Veterans’ Administration disability benefits. This proof includes an award letter of total (100%) disability, pension statement, or statement showing compensation rated at 100%.
- Proof of pension for a non-military service-connected disability.
- Proof of Railroad or Civil Service disability benefits is an award letter of total (100%) disability.
- If a taxpayer can not provide proof with one of the items listed above they will need to submit the Illinois Department of Revenue’s Form PTAX 343-A Physician’s Statement of Proof of a Disability, completed by a
physician or optometrist. The taxpayer may also be required to be re-examined by an IDOR-designated physician.
VETERANS EXEMPTION DOCUMENTATION REQUIRED

- A first-time applicant must please provide a Department of Defense DD Form 214, certified by either the Cook County Clerk’s Office or Illinois Department of Veterans Affairs. This document is not required for those reapplying.
- All applicants, must provide a Disability Certification Letter from the U.S. Department of Veterans Affairs.
- A non-remarried surviving spouse of a veteran with a disability who is eligible for the Exemption and is using the Exemption for the first time or transferring it to a new address: you must provide the disabled veteran’s death certificate and proof of ownership.
Documentation Required to Apply
- Proof of age and identity and occupancy in 2022.
- A copy of one photo ID from List A must be provided along with an Occupancy Affidavit (the affidavit is provided within the online application). The name and property address must match the application and the date of issue must verify that you occupied this property on or before January 1, 2021.

2. Proof of Property Tax Liability
- If you are listed on the deed recorded at the Cook County Clerk’s Office, this verifies your property tax liability. The Assessor’s Office staff can verify recorded deeds for you, so you do not need to provide a document.
- If you are NOT listed on the recorded deed, please provide one of the following documents to verify you are liable for payment of property taxes for this property:
- Deed (if not recorded) or contract for a deed
- Lease
- Trust Agreement
- Stock Certificate
- Occupancy Agreement
3. Total Household Income Verification
- The application will guide you through what counts as income for every member of the household.4. If the applicant was enrolled in any of the following programs in 2021, proof of enrollment in 2021 is needed. In this instance, the Income Verification doesn’t need to be completed.
- Aid to the Aged, Blind or Disabled (AABD) Program
- Supplemental Nutrition Assistance Program (SNAP)
- Low Income Home Energy Assistance Program (LIHEAP)
- The Benefit Access Program
- Senior Citizens Real Estate Tax Deferral Program


How exemptions help you save
The Cook County Assessor’s office administers property tax exemptions that may contribute to lowering your property tax bill. The exemptions listed on this page reduce the equalized assessed value (EAV), or taxed value, of a home. Exemption applications are due in early 2021 for the 2020 tax year. These savings will be reflected on the SECOND- installment property tax bill mailed in the summer of 2021.
Here’s an example of how a tax bill would be calculated, for a home with an estimated fair market value of $300,000 and a local tax rate of approx. 10%. Please note the Equalized Assessed Value (EAV) is the partial value of the property. It is the figure on which the tax bill is calcuated.
